To understand this purpose is to understand what makes a career at UMMC so rewarding and why, for some individuals, a career at UMMC is the ultimate professional achievement.Responsibilities:- Under general supervision, provide consultation, education, public relations and patient assessment services in order to obtain admissions into post acute programs.- Responsible for providing detailed clinical assessments to determine patient appropriateness for post acute inpatient programs.- Work closely with UMMC case managers, physicians, therapists and other team members to facilitate patient transfers to either Kernan, USH,or Rehabilitation Center at MD General. Also, responsible for obtaining insurance authorizations.Qualifications:- Graduation from an accredited school of Nursing or Therapy.- Current licensure as a registered nurse by the Maryland State Board of Examiners Nurses.- At least two years as a liaison or case manager preferred.- Excellent clinical evaluation skills.
