In addition, special projects/assignments requiring expertise in post acute care and alternate site discharge planning will be assigned by the Department Administrator or Director of Nursing and the Nurse Manager.Job Responsibilities: Completes and verifies accuracy of all MDS documentation for all Subacute patients as required by Prospective Payment System (PPS)/Multiple Data Set (MDS) and the Omnibus Budget Reconciliation Act (OBRA). Responsible for the MDS calendar assuring regulatory compliance. Coordinates Assessment Reference Dates (ARD) with interdisciplinary care team for appropriate classification. Maintains a trained MDS back-up team to provide coverage for RNAC/MDS nurse (i.e., PTO, leave, etc). Conducts daily MDS stand-up rounds on the Subacute Unit. Provides input into creation/revision of written policies and procedures related to PPS/MDS in accordance with government regulations to assure continued compliance. Makes routine audits of medical records documents to assure that established CAE Management and MDS/PPS policies and procedures are being implemented and followed. Assures MDS data is in agreement with medical record documentation. Has final sign off verifying accuracy for MDS data. Submits MDS data to the Ohio Department of Health (ODH) and Centers for Medicare and Medicaid Services (CMS) according to established regulatory schedule. 11) Performs other duties as assigned. Monitors rehabilitation therapy to assure timely and efficient rehabilitation evaluation and treatment sessions. Responsible for the coordination of Resource Utilization Group Scores (RUGS), patient Plan of Care and clinical staff documentation. Provides information necessary for clinical and financial reports required to monitor reimbursement and quality of care. Other duties as assigned.Education: Graduate of an accredited school of Nursing, Bachelor's degree in Nursing preferred. Strong clinical knowledge, understanding disease processes and appropriate interventions. Knowledge of home care services, financial, reimbursement and admission criteria for home care referrals preferred.Experience: Two years clinical experience, to include one year in a post acute care facility. Computer skills with data base implementation and word processing strongly preferred.Licensure/Certification/Registration: Licensure as a Registered Nurse in the state of Ohio.Physical Requirements: Sedentary Work - Exerting up to 10 pounds of force occasionally (Occasionally: activity or condition exists up to 1/3 of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body.
