ASN or BSN preferred. Education and training in Utilization Management, Core measures and Quality indicators preferred.Experience: Five (5) years experience as a nurse in the clinical setting, preferably hospital experience. One (1) year experience in healthcare Utilization Management/Utilization Review preferred. Training to be provided.Licensure: Current non-restricted RN license in the State of Georgia. Knowledge/Skills/Abilities: Must be familiar with the patient flow process from admission to discharge. Working knowledge of medical charts and the required documentation. Must be familiar with Core Measures and Quality Indicators. Must have exemplary communication skills and organizational skills. Must be able to process multiple lines of data and ability to prioritize patient needs.Responsibility #1: Organizes and drives the discharge planning process from the time of admission to the assigned unit to discharge. Rounds daily with the Staffing Flow Facilitator and medical staff. Communicates current status of patients. Evaluates and adjusts patient plan of care (POC) to facilitate outcome goals. Assists with transfers between units and adjunct facilities to meet level of care needs for the patient. Organizes and facilitates Grand Rounds once weekly on patients requiring extended care or services. Communicates with medical, nursing and ancillary staff in order to evaluate care delivery and adjust POC to meet patient needs and facilitate patient discharge planning. Facilitates the clinical education needs of patients and families to ensure continuity of care upon patient discharge.Responsibility #2: Communicates with Care Coordination Center. Reviews utilization goals and communicates with case management the status of each patient. Monitors the Length of Stay and Utilization Review status of each patient and communicates updates with the Staffing Flow Facilitator and the medical staff.Responsibility #3: Monitors Core Measures Compliance for quality compliance. Reviews charts for Core Measures Compliance and quality improvement standards compliance. Makes adjustments or changes in real-time as needed to meet the outcome goals. Completes variance reviews on quality issues that affect patient care with assigned patients. Communicates improvement needs to Director.
