The Physical Therapist (PT) is assigned to the Geriatrics and Extended Care Service Line (G&EC) and works collaboratively with PM&R Services to provide physical therapy services to the veteran population enrolled in the Home Based Primary Care (HBPC) Service located at the Lewisburg, WV Community Based Outpatient Clinic (CBOC). This position involves regular independent travel to patients' homes. The PT is responsible for evaluating the patient's functional, physical, and/or psychosocial abilities while considering the patient's capacities, the activities, and the environments in which these activities occur. The PT will evaluate the home environment with regard to safety and accessibility issues. The PT will develop a treatment plan directed toward achieving the maximum level of functioning within the home and developing a plan in collaboration with the patient and care giver. The PT will document in the medical record within specified timeframes, formats and standards. Treatment will take place within the home setting while maintaining privacy and confidentiality as appropriate. The PT functions autonomously in carrying out patient education, specialized treatments, and applying therapy procedures for which competency has been established and in accordance with laws or regulations; modifying the treatment plan, therapeutic activities and interventions based on the changing needs, goals, and performance of the patient; modifying the treatment plan when either the established goals or timeframes for achievement of those goals has been obtained; accurately documenting therapy services in the medical record within timeframes, formats and standards established by HBPC Services.
