To achieve this goal, we are currently seeking an experienced Registered Nurse (RN) to serve as our:Ambulatory Nurse Care Coordinator, Pediatrics at the Harbor (PATH)This position offers the well organized, detail oriented candidate an opportunity take the lead in ensuring the continuity and consistency of care which extends beyond practice boundaries. Duties include, but are not limited to:- Serving as a liaison to acute care hospitals and post-acute care services to facilitate comprehensive discharge planning and follow-up care.- Identifying patients needing disease management intervention and developing a care plan and treatment goals with the patient and/or family.- Identifying self management resources including health education classes and community resources aimed at achieving care plan goals; working in collaboration with Ambulatory leadership in formulating an agenda for attaining and sustaining NCQA certification; reviewing and analyzing data related to clinical care, and coordinating special quality improvement projects and studies.The primary care practice utilizes a team-based approach to care to facilitate health promotion and disease management services. The Care Coordinator will serve as a valuable member of the interdisciplinary team. The position offers a full-time, primarily day shift schedule.The PATH's population is primarily local children, ages newborn to 18 years of age. The PATH is home to a large Continuity Clinic, providing education to our Pediatric, and Medical pediatric residents. The PATH also provides clinical research and study resources, and is open Monday - Friday from 8 am - 5 pm.The successful candidate will hold a Bachelor's Degree in Nursing, or demonstrate an equivalent combination of education and experience. Licensure as a Registered Nurse in the State of Maryland, or eligibility for licensure in the State, is required.
