RN Case Manager TMC Location:-1680189Description:Coordinates patient care with physicians, patients/ significant other and members of the multidisciplinary team; and collects and evaluates information to facilitate care by:1. Assuring that admissions, continued stays and ancillary services are medically necessary, cost effective and provided in the appropriate setting to qualify for reimbursement.2. Intervening as needed to assure that care is coordinated, timely and cost effective.3. Performing concurrent screening for quality and risk management issues.4. Performing early identification of discharge planning needs, early referral of cases needing psychosocial assessment and intervention of Social Services.QualificationsBSN required.Experience should include 3-5 years of clinical practice with experience acquired in assigned speciality required. Prior experience in quality/utilization management and/or discharge planning preferred. Computer skills required.
