Job Title: RN Case Manager - Temporary PositionLocation: Newark, DEGeneral Description: This position is a casual temporary position with a duration of 12 weeks. The hours of the position are Monday - Friday 8am - 4:30pm.DUTIES: Through a collaborative process with nursing and the pts. physician, the case manager facilitates resource utilization and transition planning for a population of patients. Applicants must demonstrate exceptional communication skills and the ability to work in a fast-paced, demanding acute care hospital setting.EDUCATION & EXPERIENCE: A minimum of 3 years adult med-surg experience. Bachelor's-prepared RN with DE license and with case mgt, UM, or clinical documentation experience. Clinical experience in an acute care hospital setting is required. Candidate must provide demonstrated ability to work in a collaborative, team-based, fast-paced hospital environment.
