Performs admissions and assumes responsibility for managing a caseload of patients; maintains timely communications with others involved in the care of the patient. Provides supervision to LPNs, home care aides and family/caregivers involved in the care of the patient. Participates as a member of a patient -focused interdisciplinary care team.MINIMUM QUALIFICATIONS:Education, Knowledge, Skills and AbilitiesGraduate of an accredited Nursing program.Required Length and Type of ExperienceAt least 1 year current clinical experience as a registered nurse in an acute medical-surgical setting required. New RN grads will only be considered if currently employed as a LPN with Cleveland Clinic Home Care.Required Licensure, Certification or RegistryCurrent licensure as a RN in the State of Ohio, CPR Certification, Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter. Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter.Required Physical and Environmental DemandsMust be ambulatory and able to drive to patients’ homes, able to climb stairs, pull, push, and facilitate movement of the patient. Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds. Must be able to read, write and communicate with patients .Must have correctable vision and correctable hearing to be able to hear normal conversations. Must be able to work in settings with infectious diseases.
