Reassess to meet patient and family needs, every shift and as appropriate.Participate as an effective member of the patient care team contributing to the efficient operation of the unit.Make unit assignments according to patient acuity and by unit guidelines and/or unit standards.Evaluate and document on nursing progress notes the effectiveness of nursing interventions in relation to all patient goals/outcomes.Identify self to patient/family as the patient's nurse within one hour after shift report.Perform patient assessment on admission and document within time designated in unity standards.Initiate, prioritize and individualize written care plan utilizing age specific and developmentally appropriate criteria and unit specific standards of care.Collaborate with discharge planner or social worker, and patient/family in the development of a discharge plan.Identify patient problems from collected data, formulating nursing diagnoses within twenty-four (24) hours of admission.Initiate prioritized nursing interventions that are age specific and developmentally appropriate and document on chart each shift.Revise and document on patient care plan changing patient needs.Promote teamwork by clarifying duties, supervising personnel and assisting as needed to ensure satisfactory completion of assignments.Demonstrate responsibility for maintaining environmental safety.All documentation needs to be completed by the end of shift.Initiate patient/family teaching plan and document.Observe and assess each assigned patient each shift.Identify abnormal data and report to appropriate health team member within an appropriate time frame.Demonstrate a wide variety of clinical competencies necessary to implement nursing interventions and physicians orders.Participate in Quality Assurance audits.Provide for clean patient environment and clean and well-stocked nursing area by end of shift.Perform all procedures in a safe manner and in accordance with standard hospital policy and procedure manuals, using proper body mechanics and Universal Precautions.Identify and record patient/family teaching needs as documented on nursing care plan.Identify and record patient/family education and discharge instructions as documented on nursing care plan.Demonstrate appropriate pre and post-op care for each individual patient and document on appropriate chart form(s).Attend at least 50% of staff meetings and initial all minutes of staff meetings not attended.Attend all mandatory in-services or view videotapes of in-service.Demonstrate the ability to appropriately use and "troubleshoot" all equipment used in the area.Coordinate the patient care provided by other disciplines of the health care team.Attend annual skills training and competencies.Ensure that timecard is completed at the end of each pay period.Implement physician orders in a safe and conscientious manner.Administer accurately all medication including IV fluids.If per diem employees do not work within a three month period, this will be an automatic termination.Document drug allergies in all appropriate places upon patient admission.Document reason and effects of PRN medications on nursing progress notes.Demonstrate and maintain ability to perform basic emergency care basic life support (BLS).Assist the patient, family and significant others with the grieving process.Basic Qualifications: Must be licensed Registered Nurse in the State of Minnesota. AA, Diploma or bachelor of science in nursing (BSN).Other Qualifications: Mandatory basic life support (BLS) upon hire and advanced cardiac life support (ACLS) within 6 months of hire and kept updated.
