DescriptionNeuro and/or IMC experience preferred, will consider new graduate.Independently provides and coordinates care provided to patients within a clinical specialty. Serves as a fully effective and participative member of the multidisciplinary patient care services team. Demonstrates and models the full range of nursing professional practice in both care provided and in participation in development of strategies to improve patient outcomes, satisfaction with service and cost effectiveness at the unit level. Is accountable to patients and families for the processes and outcomes of care during an episode of care. Is accountable to peers for the advancement of group practice on the unit, and to the nurse manager for the totality of work performance.Clinical Practice/Patient CareModels high level of clinical competency and expertise related to patient populations in areas of assignment. Participates in developing/revising standards of practice to meet changing patient population requirements.Serves as a resource in planning care for complex patients. Identifies need for and initiates patient care conferences. Assumes leadership role in peer review of nursing process.Models relationship skills with patients and families to not only prevent poor outcomes and complaints, but to promote patient satisfaction.Models patient/family advocacy during real or perceived crisis. Provides leadership and facilitates appropriate response from peers during emergencies.Models effective documentation practices. Develops strategies to ensure complete documentation of patient care.Identifies impact of new technology on patient care outcomes and on practice/cost of care. Identifies and addresses staff learning needs related to patient care technology.Effectively uses and adapts a variety of teaching resources in meeting learning needs of patients/families. Identifies needs for and participates in acquisition/development of additional teaching resources. Models effective teaching and documentation.Facilitates collaboration among health team members. Takes the lead in assuring continuity and consistency of care which extends beyond unit boundaries.Identifies and addresses issues related to patient information management, including confidentiality problems.Patient Care ManagementIdentifies the need for multidisciplinary services; initiates and tracks referrals as appropriate.Takes the lead within the multidisciplinary team in monitoring patient progress, identifying problems and facilitating changes in treatment approach to meet patient care objectives.Identifies ways to improve the discharge planning process to reduce length of stay and prevent delays in discharge. Monitors patient progress towards discharge and takes the lead with colleagues in facilitating timely discharge.Actively participates in the development and implementation of clinical paths. Oversees unit level implementation for selected paths.Models highly effective interpersonal and communication skills. Responds effectively to conflictual or complex issues in coordinating services on behalf of the patient/family.Monitors competency, documentation and outcomes and counsels non-licensed staff regarding performance. Identifies ways to facilitate working relationships among all levels of staff to meet patient care requirements.Effectively sets priorities for patient care and unit needs. Coaches and assists staff to manage time effectively.Takes a leadership role in incorporating diverse payor requirements into patient care processes at the unit level. Monitors activities, including referrals to assure that requirements are met.Performs related duties as assigned.
