Works in conjunction with physicians, nursing staff and ancillary staff to evaluate, propose, plan and perform patient assessment and treatments. Collects data regarding the patient's physical, psychosocial and health history; organizes the data into the formulation of a plan of care; identified any additional information needed and makes care decisions based on identified needs. Provides care that is specific to the patient care setting, identified needs and plan; ensures that care provided is appropriate to the severity of disease, condition, impairment, or disability. Facilitates the process for the provision of access to the appropriate level of care and services based on the patient's identified needs. Assists with discharge planning. Identified teaching opportunities and provides the resources necessary for achieving the educational goals for the patient. Exhibits professional leadership in the delivery of patient care; provides direction to staff supervised. Assists with measuring quality and outcomes on a continuous basis. Possesses appropriate interpersonal and organizational skills. Manages time and resources in a manner that meets patient and departmental goals. Registered Nurse, Occupational Therapist, Physical Therapist, or Certified Wound Ostomy Continence Nurse (CWOCN). Current non-restricted Georgia license as Registered Nurse, Occupational Therapist, or Physical Therapist. Basic clinical skills with exhibited intellectual ability to apply skills and knowledge to practice. Prefer two (2) years in a clinical setting that provides wound care to both acute and chronic patients. Experience in diagnostic testing and case management helpful. CPR (required), ACLS (preferred).
